Criteria for the Georgia Pediatric Program (GAPP):

✔  Age Requirement – Must be under 21 years old.

✔ Medical Necessity – Must require medically necessary

nursing care and/or personal care support services.

​

✔ Medicaid Eligibility – Must be eligible for Medicaid or have Medicaid pending.

​

✔ Residency – Must be a resident of Georgia.

​

✔ Physician’s Orders – Must have a medical evaluation confirming the need for personal care or supportive services.

​

✔ Application Review Process – Eligibility is reviewed by the Medical Review Team, typically within 30 days.
